CHARTING symptoms on 4-day Rotation

A chart may help to see progress, to share with your healthcare provider.   

You can PRINT this page, to work from a hard-copy of chart  ie, by your bedside for nightly scoring AT SAME TIME EACH DAY.

OR Copy and paste chart to ie. a Notes Program to score there daily(BUT PRINT WHEN FINISHED 4 Days,  to keep chronological file to see if scores improve with each Rotation.  Clear chart, EXCEPT LEFT COLUMN to start a new 4-Day Rotation.

The top 4 symptoms stay the same for minimum 2-3 Rotations.  General Well-being scores may indicate a trend earlier.  Health provider may use Chart data to help sort out problem foods, ie. if  Day 2 always has low scores, Day 1 and Day 2 foods may be reviewed, although other factors may also be considered.

Score ALL each Day.  Score ALL of your unique 4 top symptoms, as well as ALL 9 Generally Wellbeing categories each day, so that total scores can be meaningfully compared.  (Missing a Day or two may be OK to see a trend)

DAY 1 on your very first chart is your BASELINE : Flip back to it ‘s hard-copy  in your file to remind yourself that for your Top issue #1, a  “4”  means waking 3x a night.

Date: Day 1  (1- 10 best)  Day 2   Day 3   Day 4 
#1 ie. let’s say your top issue is sleep 4= (ie. waking 3x a night
#2 ie. let’s say your second top issue is rash 3= (ie. itch and and burn on inner wrists)
#3 ie. headache 4= (ie. headache 2x /week)
#4 ie. knee joint pain  8= wakes me at night
Compliance with diet %   70%      
Unusual events  ie. bad stress day at work
Daily Sum of above  19      
 
General Wellbeing:
Sleep quality  2= (ie. tossing)
Appetite
Positive mood
Energy level
Patience/calm
Meal digestion
Gas
Stool
Clear thought
Overall Daily Total
